4.13 Hazard & Risk Identification, Assessment, Prevention and Control

LBTO is committed to performing work safely and with minimal impact on people, property or the environment. Effective safety and risk management relies fundamentally on the successful identification and assessment of hazards and risks during the lifecycle of a system so to identify and implement corrective actions that reduce the likelihood of a mishap from occurring. This includes determining and evaluating the organization’s sources, situations, acts, activities, and data inputs to the hazard/risk analysis process. This process relies heavily on the knowledge, skills, experience, and abilities of the personnel. As hazards and risks are identified, they are entered into the hazard/risk tracking system, mitigated to as low as reasonably feasible, and/or accepted by the organization.

Effective mitigation relies on implementing effective controls to protect workers from workplace hazards; help avoid injuries, illnesses, and incidents; minimize or eliminate safety and health risks; and help employers provide workers with safe and healthful working conditions. Controls should be selected based on the National Institute of Occupational Safety and Health (NIOSH) Hierarchy of Controls:


Assessment of Hazards and Mitigation of Risks

The supervisor, worker, project manager, and contractor share responsibility to identify and control hazards and risks associated with the operations and equipment under their control or within their work scope. Mitigation measures must be in compliance with all applicable international, federal, state, and local code and regulations, including (at a minimum) federal regulations (OSHA, EPA, DOT, etc.), and with the requirements of the LBTO HSE Manual and University of Arizona requirements. Tools and training are provided by HSE to assist in this this process, which are linked under "Forms and Tools" and "Other Program Documents and Resources" sections below.

Additionally, there is a formal methodology used to identify, assess, control, and track hazards and risks that is outlined in LBTO's Hazard Identification & Risk Assessment Standard M004s00830.
